Bring a list you can trust
Check the names and amounts on your current medication containers, including over-the-counter products and supplements. Record what you actually take and questions about missed doses or side effects. Discuss changes with your clinician rather than stopping, restarting or changing medication on your own.
Connect the records
Relevant test results and notes from other clinicians can help make a follow-up more informed. Upload a readable PDF or image through the appropriate secure intake step. Check the selected patient before adding a record, particularly when arranging a visit for a dependent.
If your clinician has asked you to monitor readings at home, bring the record and explain how you collected it. Measurements are useful context; the clinician decides what they mean and whether more evaluation is needed.
